Community First Services and Supports (CFSS) is a Minnesota health care program that provides flexible options for individuals who need assistance with daily living and other approved support needs. CFSS can include assistance with activities such as personal care, eating, bathing, grooming, mobility, meal preparation, household tasks, medication assistance, and other approved health-related or community-based supports.
Our goal is to provide dependable, person-centered support that respects each individual’s needs, preferences, and independence. We work with individuals and their families to help ensure that care is provided with dignity, compassion, and respect.
Supporting Independence at Home
CFSS offers individuals greater choice and flexibility in how their supports are arranged. Depending on their approved service plan, individuals may receive services through a CFSS provider agency and may have additional options for managing their care and supports.
